Hospital & Activities Reports are due by the 10th of each quarter for the previous quarter.  Reports submitted after the 10th will not be credited until the following quarter's reporting period.  Reports should be submitted for the period from the 1st day of the first month to the last day of the third month in in the quarter inclusive. 

Monthly Hospital & Activities Reports may be viewed online thru the NJ State Portal
